The Mission of Care Model

Nutrition has been our main focus during the last 12 years. We continue to support many food insecure children, adults and the elderly each month. We facilitate nutrition rehabilitation at the household level, within the home of each client. Each person receives a monthly food package, medical care and wellness information as needed.

Addressing the community complex challenges that impact on individual lives requires multiple players. We are not about getting people more involved in community service programs, but getting programs and health services more involved in what the family is already doing to improve their well-being.

We coordinate individual support resources by bringing the nutrition, medical and social support teams to the individual in their home setting.

Our Mission of Care Model is guided by the following fellowship practices.

  • Visits are voluntary.
  • Community outreach volunteers are professionally and continuously trained.
  • We engage the immediate neighbors of each family for continued support.
  • We always keep our promise to “come again”.
  • We always bring a gift with us – from a few handfuls to lots of kilograms of food.
  • We do not introduce program activities, we introduce and connect people.
  • We enjoy a good long conversation and shared cup of tea as often as possible.
  • Home visit teams go in groups and debrief to the larger team after each visit.
  • We always start from lessons previously learned to plan and implement the next visit.
