The Mission of Care Model

Nutrition has been our main focus during the last 12 years. We continue to support many food insecure children, adults and the elderly each month. We facilitate nutrition rehabilitation at the household level, within the home of each client. Each person receives a monthly food package, medical care and wellness information as needed.

Training

Uniquely tailored sessions engage community outreach volunteers with increased understanding of the program identity, the patients/clients we serve, and their role as extended health promoters. They learn how to build trust with families/communities, and they increase their capacity to better engage families/communities through temporal assistance. Theses trainings are interactive, dynamic, and essential to the start of an evidence-based homecare visit and community engagement practice.

Training options include: 1) homecare visit and community crisis care training, 2) partner implementation support, 3) partnership accrediting.
